Care Coordinators
Care co-ordinators help to co-ordinate care across the health and care system, helping people make the right connections, with the right teams at the right time. They are effective in bringing together multidisciplinary teams to support people’s complex health and care needs, particularly those with long term conditions, multiple long-term conditions, and people living with or at risk of frailty.
Care Coordinators work closely and in partnership with GPs and other primary care professionals within Primary Care Network (PCN) to support patients to use health services appropriately and effectively. They focus on delivery of personalised care to reflect local PCN and national health priorities, or to support groups of patients identified through risk stratification. Care Coordinators can also support PCNs in the delivery of enhanced health in care homes.
This role may lead to training opportunities for other health or social care jobs, including nursing or one of the Allied Health Professions.
Is there any recruitment funding available to Primary Care Networks?
This is a role that is included in the Additional Roles Recruitment Scheme from April 2020. Primary Care Networks may therefore be able to access funding to cover salary and on costs. You can find out more by referring to the information in the links below:
